MONONEUROPATÍA CRANEAL MÚLTIPLE COMO FORMA DE PRESENTACIÓN DE NEUROSARCOIDOSIS

Resumen

Se presenta el caso de un paciente femenino de 57 años de edad con antecedentes de hipotiroidismo, que ingresa por cuadro de mononeuropatía craneal múltiple con afectación de pares craneales(PC) II, III completos, IV, rama oftálmica del V y IX. Se realiza diagnóstico definitivo de neurosarcoidosis (NS) por biopsia meníngea.

El objetivo de esta publicación es reportar el caso de una sarcoidosis con afectación exclusiva delsistema nervioso (SN), debido a lo excepcional de su presentación. Se debe sospechar y planteardentro de los diagnósticos diferenciales a todo paciente que se presente con cuadro de mononeuropatía craneal múltiple.

MULTIPLE CRANIAL MONONEUROPATHY AS A PRESENTATION OF NEUROSARCOIDOSIS 

abstract

We report the case of a 57-year-old female patient with a history of hypothyroidism, presenting with multiple cranial mononeuropathy involving cranial nerves II, III (complete), IV, the ophthalmic branch of V and IX at admission. Patient was finally diagnosed with neurosarcoidosis (NS) following meningeal biopsy.

The purpose of this work is to report a case of sarcoidosis involving only the nervous system, due to its extremely rare presentation. A high degree of clinical suspicion is needed for diagnosis, and sarcoidosis  should be part of the differential diagnoses in a patient presenting with multiple cranial mononeuropathy.
