PLASMOCITOMA ÓSEO SOLITARIO COXOFEMORAL. PRESENTACIÓN DE UN CASO
SOLITARY COXOFEMORAL BONE PLASMACYTOMA: CASE PRESENTATION
Resumen
Los plasmocitomas son tumores neoplásicos infrecuentes, monoclonales de las células plasmáticas, ubicados fuera de la médula ósea, más frecuentes en el tracto respiratorio superior y con muy baja frecuencia retroperitoneal. Este artículo aborda el caso de un paciente de sexo masculino, de 65 años de edad, que presentaba fiebre y dolor coxofemoral derecho. En el examen físico se encontró un soplo en el foco mitral y dolor ante la movilidad activa y la pasiva de la articulación coxofemoral. Se realizó una tomografía axial computarizada abdominopélvica que evidenció tumor retroperitoneal con afectación ósea. Se intervino quirúrgicamente con la exéresis del tumor; la biopsia reveló un plasmocitoma coxofemoral. El tratamiento consistió en una cirugía más radioterapia, que evidenció una complicación hematológica con pancitopenia grave. En cuanto al plasmocitoma, se han detallado en la literatura varios casos de tumores solitarios en el área retroperitoneal, de los cuales menos del 5% fueron como los describe Weber, y se complicaron con falla renal o alteraciones hematológicas o neurológicas. Este paciente tuvo un proceso infeccioso (endocarditis) asociado, por lo que presentó anemia e insuficiencia renal, las cuales fueron superadas.

ABSTRACT
Plasmacytomas are rare, monoclonal neoplastic tumors of plasma cells located outside the bone marrow, more frequent in the upper respiratory tract, with very low retroperitoneal frequency. This case is a 65-year-old male patient admitted due to fever and right hip pain. Physical examination was found in the mitral focus, and pain from the active and passive mobility of the coxofemoral joint. Abdominal pelvic CAT was performed, where retroperitoneal with bone involvement is evident. Surgery was performed with excision of the tumor, the biopsy reporting coxofemoral plasmacytoma. The treatment consisted in surgery plus radiotherapy, revealing haematological complication, with severe pancytopenia. Several cases of solitary tumors in the retroperitoneal area have been described in the literature, being less than 5% as described by Weber, with haematological or neurological alterations complicating with renal failure. The patient underwent an infectious process (endocarditis) associated, so anemia and renal failure were present, which were overcome.
