Por lo general, se deben a la transferencia de líquido transdiafragmático exudativo durante un episodio de pancreatitis aguda y se resuelven espontáneamente. En raras ocasiones, en pacientes con pancreatitis crónica, los derrames pueden derivarse de una conexión fistulosa entre el páncreas y el espacio pleural, lo que se conoce como fístula pancreático-pleural (FPP). Se presenta el caso de un paciente con antecedente de pancreatitis crónica y que requirió en la última internación la colocación de un stent en el conducto de Wirsung; en esta oportunidad ingresa por una reagudización del cuadro pancreático con agregado importante de derrame pleural derecho. El análisis fisicoquímico de este determinó la presencia de altos niveles de amilasa; la utilización de imágenes contrastadas permitió establecer la presencia de FPP derecha. A RARE CAUSE OF PLEURAL EFFUSION.A CASE REPORT</strong></p><p><strong>Abstract</strong></p><p>Pleural effusions in pancreatitis are usual. They generally originate in the transfer of exudative transdiaphragmatic fluid during an acute pancreatitis event, and resolve spontaneously. Rarely, in chronic pancreatitis patients, effusions can result from a fistulous connection between pancreas and pleural space, which is known as pleural pancreatic fistula. In this case, a patient presents with a history of chronic pancreatitis that required hospitalization and stent placement in the duct of Wisung; now he is admitted for exacerbation of his pancreatic condition with the addition of important significant right pleural effusion. Physico-chemical analysis of it showed high levels of amylase; the use of contrast images allowed us to confirm the presence of right pancreatic-pleural fistula. In this article, we provide the key elements for suspicion, diagnosis and treatment.</p><p><strong>Key words</strong>.
