CRIPTOCOCOSIS MENÍNGEA ASOCIADA A LUPUS ERITEMATOSO SISTÉMICO. A PROPÓSITO DE UN CASO

Resumen

La criptococosis es una enfermedad infecciosa micótica sistémica, ocasionada por el complejo Cryptococcus neoformans/Cryptococcus gattii, que afecta a las personas inmunodeficientes e inmunocompetentes, respectivamente. La vía de acceso del agente al organismo es la respiratoria, cuando la persona inhala las levaduras junto con polvo o tierra que contiene heces de aves (principalmente, de palomas). El diagnóstico se realiza mediante la observación del hongo, aplicando estudios histopatológicos, con cultivos y a través de la presencia del antígeno capsular. La infección puede comprometer diferentes órganos –por ejemplo, el pulmón–; sin embargo, la más grave se constituye en el sistema nervioso. Las infecciones del sistema nervioso central solamente constituyen el 3% de las infecciones que se presentan en pacientes con lupus eritematoso sistémico (LES) debido a que estos individuos tienen mayor riesgo de contraer este tipo de enfermedades por el uso agresivo de corticoesteroides.
Se presenta el caso clínico de un paciente con antecedentes de LES con un deficiente manejo actual, que comienza con cefalea y alteración del estado de conciencia debido a la alta dosis de corticoides que recibe. Se lo considera inmunodeficiente, por lo que se sospecha clínicamente una infección oportunista; se procede a realizar los estudios diagnósticos enfocados, tanto de laboratorio como de imagen, y se llega al diagnóstico final de una meningitis atípica criptocócica, y la consecuente implementacióndel tratamiento correspondiente.

ABSTRACT
Cryptococcosis is a systemic infectious fungal disease, caused by the Cryptococcus neoformans/Cryptococcus gattii complex, affecting immunocompromised and immunocompetent persons,respectively. The agent accesses the organism through the respiratory system, when the individual inhales the yeasts together with the dust that may contain feces of birds, mainly of pigeons. The diagnosis is made by observing the fungus, applying histopathological studies, with cultures and the presence of the capsular antigen. The infection can compromise different organs, such as the lungs; however, the most serious one is in the nervous system. Infections of the central nervous system (CNS) only constitute 3% of the infections that occur in patients with systemic lupus erythematosus (SLE), since these patients have a higher risk of contracting this type of disease as they receive an aggressive use of corticosteroids.
We present the clinical case of a patient with a history of SLE with poor current management, who first presented headache and an altered state of consciousness. Due to the high dose of corticosteroids he receives, he is considered immunocompromised, so we clinically suspected opportunistic infection, and proceeded to perform diagnostic studies focused on both laboratory and imaging; with which we finally arrived at the diagnosis of atypical cryptococcal meningitis, thus implementing its consequent treatment.
