TUMOR FILOIDES DE LA GLÁNDULA MAMARIA

Resumen

El tumor filoides es una neoplasia poco frecuente de la glándula mamaria, representa entre 0,3 y 0,9%
de la totalidad de los tumores de la mama y de 1 a 2,5% de los tumores fibroepiteliales. Debido a la
baja casuística y la poca experiencia en el diagnóstico y tratamiento de estas lesiones, comienzan
a utilizarse numerosas nomenclaturas. Desde el punto de vista histopatológico, el comportamiento
biológico del tumor filoides viene definido por el número de mitosis por 10 campos de gran aumento.
Tienen un comportamiento biológico impredecible; hay algunas características histológicas que se
solapan entre las lesiones fibroepiteliales y una tendencia a la progresión al fenotipo maligno. La mamografía
y la ecografía son los pilares diagnósticos de rutina en los tumores de la glándula mamaria;
el tumor filoides a menudo imita a un fibroadenoma en la mamografía. Por lo general la tumoración se
presenta con bordes bien delimitados y no es dolorosa. El complejo areola pezón puede aplanarse
por la presión que ejerce el tumor y es característica la presencia de una red venosa colateral por
ectasia de las venas de la piel cercanas al tumor. El tratamiento usual de los tumores filoides es la
cirugía con márgenes de 1 cm.

PHYLLODES TUMOR OF THE MAMMARY GLAND

Abstract
Phyllodes tumor is a rare neoplasm of the mammary gland, representing between 0.3 and 0.9% of
all tumors of the breast and 1 to 2.5% of fibroepithelial tumors. Due to the low casuistry and the little
experience in the diagnosis and treatment of these injuries, numerous nomenclatures are beginning to
be used. From a histopathological perspective, the biological behavior of the phyllodes tumor is defined
by the number of mitosis by 10 high-power fields. They have an unpredictable biological behavior;
there are some histological characteristics that overlap between these fibroepithelial lesions and a
tendency towards a malignant phenotype progression. A mammography and an ultrasound are the
routine diagnostic tools in mammary gland tumors; the phyllodes tumor often mimics a fibroadenoma
on a mammography. In general, the tumor appears with well-defined edges and it is not painful. The
nipple-areola complex can be flattened due to the pressure exerted by the tumor, the presence of a
collateral venous network by ectasia of the veins of the skin near the tumor being characteristic. The
standard treatment for phyllodes tumors is surgery with margins of 1 cm.
