CONTROVERSIAS EN LA INDICACIÓN DE TRATAMIENTO DE PACIENTES CON ENFERMEDAD DE FABRY EN LA ARGENTINA
Resumen
Introducción. Los criterios de indicación de tratamiento en enfermedades lisosomales son motivo de
controversia. La detección temprana de síntomas posibilitaría un inicio temprano de tratamiento y mejoraría
el pronóstico de pacientes afectados, aunque esto es dificultoso, por su presentación clínica variable.
Objetivo. Analizar los criterios de indicación de tratamiento de pacientes con enfermedad de Fabry
(EF). Material y métodos. Estudio observacional, de diseño transversal. Resultados. Se incluyó a 73
pacientes (26,43 ± 16,54 años); 31 varones y 42 mujeres; 24 pediátricos y 49 adultos; de 14 genotipos.
15 pacientes caso índice (CI); 13 varones y 2 mujeres; todos tuvieron indicación de tratamiento. No hubo
CI pediátricos. De 26 pacientes “no casos índice” (no CI), 17 fueron adultos (7 varones; 10 mujeres) y
9 pediátricos (5 varones; 4 mujeres). En la población CI sólo un paciente no presentó manifestaciones
tardías de la EF como criterio de iniciación de tratamiento; su indicación fue compromiso del sistema
nervioso periférico (SNP). El resto de los pacientes presentaron al menos una manifestación tardía de EF
como criterio. Entre ellos, 46,6% presentaron dos manifestaciones tardías; 40,00%, tres manifestaciones
tardías, y 6,00%, una manifestación tardía de EF. De 26 pacientes no CI, que tuvieron indicación de tratamiento,
76,9% ya presentaron al menos una manifestación tardía de EF; 38,4%, una manifestación tardía;
23,07% dos manifestaciones tardías, y 15,4%, tres manifestaciones tardías de EF. Conclusiones. Las
recomendaciones de inicio de tratamiento basadas en la presencia de manifestaciones clínicas de la EF
deberían ser revisadas, debido a que probablemente signifique comenzar tratamiento en etapas tardías.

CONTROVERSIES IN TREATMENT INDICATION OF FABRY DISEASE PATIENTS
IN ARGENTINA
Abstract
Introduction. Criteria for the indication of treatment in lysosomal diseases are controversial. The
early detection of symptoms would allow an early start of treatment and improve the prognosis of
affected patients, although this is difficult, due to its variable clinical presentation. Objective. To
analyze the criteria for indication of treatment in Fabry disease (FD) patients. Material and methods.
Observational study, transversal design. Results. 73 patients were included (26.43 ± 16.54 years);
31 male and 42 female; 24 pediatric and 49 adults; of 14 genotypes. 15 “index case” (IC) patients; 13
male and 2 female; all had an indication of treatment. There were no pediatric ICs. Of the 26 “no index
case” (no-IC) patients, 17 were adults (7 male, 10 female) and 9 pediatric (5 male, 4 female). Among
the IC population, only one patient did not present late manifestations of FD as criteria for initiating ERT;
its indication was compromise of peripheral nervous system (PNS). The rest of the patients presented
at least one late manifestation of FD at the time of diagnosis. Among them, 46.6% presented two
late manifestations; 40.00%, three late manifestations, and 6.00%, a late manifestation of FD. Of 26
no-IC patients, who had indication for treatment, 76.9% had at least one late manifestation of FD;
38.4%, a late manifestation; 23.07% two late manifestations, and 15.4%, three late manifestations of
FD. Conclusion. Recommendations for the initiation of treatment based on the presence of clinical
manifestations of FD should be reviewed, since it probably means starting treatment in later stages.
